Legal Crash is required by law to maintain the privacy of protected health information (PHI), provide you with this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.
We reserve the right to change our privacy practices and the terms of this notice. Changes will apply to PHI we already have about you. The new notice will be posted on our website and will be available upon request.
We may use and disclose your PHI to facilitate your medical care, coordinate with attorneys to understand the scope of your injuries, and ensure proper documentation of your medical treatment for claim purposes.
We may use your health information to evaluate your accident claim, estimate compensation, prepare case summaries for referred attorneys, and communicate with insurance companies on your behalf.
We will disclose your PHI when required by federal, state, or local law, including to law enforcement agencies, courts, and regulatory bodies as required.
For uses and disclosures beyond those described above, we will obtain your written authorization. You may revoke any authorization at any time in writing, except where we have already taken action based on it.
